The frenulum is like the joining ridge under the tongue. It is the small sheet of skin underneath the glans (head) of the penis, which joins the glans to the foreskin. If the frenulum is too short on the erect penis, it does not allow the foreskin to slide forward and causes pain. During intercourse, this can happen and as a result the tight frenulum will often rip and bleed. In the event, immediate pressure must be applied to the area and urgent medical attention is required. A few surgical sutures will be placed over the bleeding area.

Short frenulum is common and is seen in 5% of uncircumcised men between 15 and 30 year of age. Some men may go through their entire life suffering from pain and bleeding without knowing that they have this problem. Many men do not even know that there is a cure. The degree of tightness and thickness of the frenulum is very variable.

Treatment is usually by a simple surgical correction. Circumcision is not necessary.

A very thin and tight frenulum can be released with a simple surgical incision under local anaesthesia. The wound is dressed and allowed to heal over a couple of days. No stitching is required.

A thicker and tighter frenulum will require a frenuloplasty surgery. We perform this is simple procedure under local or general anaesthesia in under 30 minutes. A Y-shaped (referred to as a y-plasty) incision over the tight frenulum. When the incision is stitched together it forms a straight line, which is barely visible.

The patient can return to work within 1-2 days. Healing takes place in about two weeks and the area is considered to be fully functional in about 1 month. The stitches need not be removed as it self dissolves in a few weeks.

Penile curvature
Congenital or inborn penile curvature is caused by asymmetry in compliance of tissue in either the shaft or base of the penis. Deviation can be to the left, right, upward, downward or a combination. This condition has been reported to be present in 1% - 6% of men. Mild curvature is common and does not cause much symptoms. More significant penile angulations not only cause sexual dysfunction, difficulty and pain on intercourse or total coital incapacity, but also cause severe psychological problems. Surgical intervention to cure penile curvature is required when coital function is affected.
There are several techniques available to correct penile curvature. The popular Nesbit technique involves an elliptical excision and plication.

Our centre prefers the multiple plication technique as it is less invasive and does not require cutting or excising part of the tissue of the penis shaft. Hence, this minimizes potential damage to nerves around the penis.
Multiple Plication Surgery for Penile Curvature
This procedure is a day surgery procedure, performed under general anaesthesia. An incision is made around the glans of penis (similar to circumcision) and the skin of the shaft of the penis is pulled down.

The curvature areas are identified and multiple plication stitches are placed on the opposite side of the curvature. This acts as counter traction to pull the penis straight.

The stitches used are fine and permanent sutures. They will not break during erection and are well buried so that they cannot be felt under the skin of the penis.

The correction of curvature is examined during surgery by inducing an erection and additional plication sutures can be placed for a complete correction. The skin of shaft of penis is pulled back to its original place and the wound closed. The final appearance of the surgical wound is similar to a circumcision surgery. Stitches used on the skin will dissolve and do not require removal at a later date.

Complications are uncommon. The most notable side effect is mild penile length shortening after the surgery. Shortening is due to correction of the convex surface but the plicating stitches.

The patient can return to work after several days. The wound heals after 10 days and is sexual intercourse can resume after 6 weeks.
